When evaluating a nursing care plan, the nurse has to identify the factors that have contributed to the success or failure in achieving client goals. The entire primary healthcare team holds a conference with the client and the client's family. The purpose of this conference is to identify long-term goals that are still unresolved and to plan for continued assistance to the client. Holding this conference and identifying unresolved long-term goals are a part of the client's discharge planning and not a part of the evaluation of the nursing care plan. The nurse should communicate all aspects of care given to the healthcare team to bring about effective implementation of the nursing care plan. Communicating all aspects of care to the healthcare team will not help in evaluating the nursing care plan.
