insufficient erasing of the previous image

Rationale: The appearance of a double exposed image is the result of using the same cassette for a previous projection or is a result of the CR system insufficiently erasing the PSP.

2 inches

Rationale: Gonadal shielding must be used within 2 inches of the primary beam.
