 cracked anode
  tube overload
  blown transformer
  vaporization of the filament

Question 2

How are shielding accessories best inspected?
 
 with fluoroscopy
  with radiography
  visually
  should just be recycled every 3 years

cracked anode

Rationale: Single, large exposures to a cold anode should be avoided to prevent a cracked anode. Tubes should be warmed up according to manufacturer warm-up procedures. A sudden and large dose of heat causes the anode to expand.

Answer to Question 2

with fluoroscopy

Rationale: Fluoroscopy should be used to inspect technologist aprons, shields, and gloves. Using fluoroscopy, holes and cracks can been seen on the monitor.
