Asking how a client can prevent a heart attack from happening again is very likely to make the client's guilt over the heart attack worse because of the subtle implication that the client could have done something to prevent the previous heart attack. While having better eating and activity habits may lower the risk of a heart attack, there is no guarantee that such behavior will eliminate all risk of a heart attack. Furthermore, guilt resulting on what one could have done before to prevent the heart attack may be counterproductive.
