Answer to Question 1

C
The client's disinhibition and depression are presenting the greatest threat to the family's safety. The client in this situation refuses to allow his hunting guns to be taken out of the house. This poses a major threat to the family. As the pathology of dementia progresses, there is often a general loss of inhibition that may take a variety of forms. Episodes of physical and verbal aggressiveness may occur. The result is that the client may become harmful to self or others.

Answer to Question 2

B
In Peplau's working phase of the nurse-patient relationship, the patient begins to respond selectively to those who can help meet the felt need. In this phase, the patient begins to experience an increased sense of being as well as developing capability for problem solving alone and with others.
