Answer to Question 1

3
Rationale: Administering PRN zolpidem tartrate (Ambien) does not suppress REM sleep and is effective for sleep. Sleep should be encouraged because sleep deprivation may enhance mood disorder symptoms. Having the night nurse talk with the client does not encourage the client to rest. Sleeping during the day interferes with establishing a regular sleep pattern at night. Providing strenuous exercise prior to bedtime promotes stimulation rather than relaxation.

Answer to Question 2

4
Rationale: Children who are hit by their parents exhibit more than double the rate of violence against siblings than children whose parents do not hit them. Hitting children increases the probability that they will be violent. Parents should not be complacent about sibling aggression; siblings cause 3 of all child homicides in the United States. Most adults in a position to know about violent confrontations and relationships (parents, faculty, police, medically trained personnel) have an awareness of sibling and peer violence that is very low.
