Answer to Question 1

Ans: C
Feedback:
Nurses assist with psychiatric diagnosis by sharing important information about clients gathered during the nursing history, MSE, and daily observations. Thus, a nurse's working knowledge of the DSM-IV-TR maximizes efforts by interdisciplinary teams to help clients. Nurses do not normally contribute directly to the determination of a specific psychiatric diagnosis. Nursing taxonomy and psychiatric taxonomy are complementary, not mutually exclusive.

Answer to Question 2

Ans: A
Feedback:
The mental status examination (MSE) helps identify whether clients are experiencing abnormalities in thinking and reasoning, feelings, or behavior. It is part of the tool kit for gathering objective and observational data.
