Answer to Question 1

ANS: 4
Option 1 would cause concern that the client will hide suicidal intent, so this does not meet the criteria for discharge. Option 2 could cause concern that the client will become lonely. Option 3 is a neutral response. Only option 4 is a clear statement from the client that he is feeling more positive.

Answer to Question 2

ANS: 2
A client who has a history of suicide gestures or attempts is at greater risk for using this behavior style again. This is unsafe behavior that needs to be evaluated, so option 2 is correct. Options 1 and 4 are unprofessional. Option 3 may be an unwarranted assumption.
