Clozapine (Clozaril) may be used if the client has not responded favorably to the use of two different neuroleptics. It is not normally a drug of first resort, however, due to significant adverse effects.

The most therapeutic response to a client's or patient's delusions is to empathically acknowledge the stress that these delusions may cause and to redirect the conversation in a respectful way. Accusing the person of delusional thinking, acknowledging the reality of the delusions, and trying to convince the person that the delusions are not real are interventions that may provoke rather than relieve symptoms.
