There are several basic requirements before physician-assisted suicide can be carried out
  in the Netherlands, such as the requirement that the patient's request be voluntary and
  well-considered. One thing that is not a requirement is that
 
  a. the patient be fully informed.
  b. the patient's family (or closest relatives) give their consent.
  c. a second independent physician be consulted.
  d. all the alternatives acceptable to the patient for relieving suffering have been tried.
  e. the patient be experiencing or about to experience severe suffering.
