Answer to Question 1

Once in early summer and again in fall

Answer to Question 2

First year: Select the strongest shoot and tie it up to the bottom wire of a trellis. That winter prune off all remaining canes.
Second year: The second summer the shoot will reach the top wire of the trellis and should be tied to it. Then the tip is cut off to cause branching.
Third year: The side shoots that develop after the main trunk is tipped should be trained along the wires of the trellis, with one side shoot in each direction along each wire.
