Risk for ineffective therapeutic regime management is the diagnosis with the highest priority for the client at this time. There is no evidence to suggest the client is not coping or is experiencing decisional conflict regarding the removal of the cataracts. The client does have disturbed sensory perception; however, the postoperative care is the highest priority at this time.

Rationale:
These are indications of normal REM sleep in the newborn. The mother should be reassured that this is normal. Having the mother wait until the next checkup unnecessarily delays this reassurance. There is no need for an immediate trip to the clinic or to the emergency department.
