Answer to Question 1

3. Let's talk about other options that might work as well for you..

Rationale:
The client is the center of the team, and the goal is to facilitate healing. There are always other options to consider to reach that goal. The nurse would discuss other options with the client, which will most likely increase cooperation by the client, who will feel in control as the decision is made. By leaving the room, the nurse and doctor have turned their backs on the client. The client may not understand the recommendations, but pointing that out can be seen as demeaning. Telling the client that the doctor only wants what is best sends the message that the client does not know what is best, when, in fact, a well-informed client does and should be able to make the correct choice.

Answer to Question 2

1. Remind the client and family that the grief process is different for everyone and that there can be no time limit set

Rationale:
The client's mother is urging the client to get over the death of a spouse. The nurse should remind both the client and family that the grief process is individual and that there is no set time limit for the process to end. Agreeing with the mother would be ignoring the client's feeling. The nurse should not avoid getting involved with the family conflict if the client is experiencing an alteration from a healthy state, because that would be ignoring the client's need. Telling the client that arguing with a parent never ending in a good way is inappropriate.
