3. Increased confusion

Rationale:
Increased confusion is a symptom that results from the infection and the cognitively impaired client's inability to state feelings correctly. Hypertension is not a sign or symptom of a urinary tract infection but of chronic renal failure, medication, arteriosclerosis, as well as other conditions. Flank pain occurs with pyelonephritis; pain with urinary tract infection occurs with urination. With a urinary tract infection, fever is usually low grade.

Answer to Question 2

1. The client is returning to the precrisis level.

Rationale:
Outcomes for a victim of rape are highly individualized. The client stating that she feels like her old self and is able to socialize without the fear of being attacked is approaching a precrisis level of functioning. The client is not in denial. The client may or may not need additional treatment. There is no evidence that the client has been taking sedatives.
