Answer to Question 1

B
The cerebellum processes information from the cerebral motor cortex, which results in equilibrium, balance, and posture. The brain stem is responsible for respiratory and cardiac regulation, level of awareness, and the reticular activating system. The cerebrum is responsible for awareness, consciousness, communication, and voluntary movement. The ventricles would not play a role with equilibrium.

Answer to Question 2

3. Fatigue

Rationale:
The client states that the worst part of the disease exacerbation is being tired even though leg spasms and blurred vision are present. The nurse should identify the nursing diagnosis of fatigue as being a priority for this client. The diagnoses of impaired physical mobility because of the leg spasms and disturbed sensory perception because of the blurred vision are additional nursing diagnoses applicable for this client, but they are not the priority based on the client's statement. The client may or may not have a self-care deficit.
