Answer to Question 1

1. Intervene to reduce the anxiety after assessing for the cause

Rationale:
The nurse should intervene by assessing for the cause of the anxiety. Leaving the client alone in the room will not help with the client's anxiety. Phoning the physician may not be helpful to the client. Providing a benzodiazepine may or may not be helpful to the client. The nurse needs to intervene after assessing for the cause.

Answer to Question 2

A
The temperature of the bath or shower should be checked by using the hands and not the feet. The feet should be inspected every day. Clean cotton socks should be worn. The patient should be instructed to never go barefoot, even when at home.
