Answer to Question 1

1. I have been able to gain 5 pounds on the new diet..

Rationale:
The elderly client with cholecystitis is at risk for infection. A goal would be to stabilize or increase weight through the appropriate diet to support the immune system and resist infection. Exercise is excellent but does not meet this client's goal. Again, the client is dating, which is good, but, it would not be a goal for cholecystitis. The client would want to decrease fat intake.

Answer to Question 2

4. Spiritual strength

Rationale:
The client is expressing hopefulness about the future, which is evidence of spiritual strength. The client is not demonstrating denial, apprehension, or conflict about the future.
