Initially, evaluation of community health care was not well-defined or controlled. In 1991, the American Public Health Association published national standards and objectives in Healthy People 2000 . Then the APHA updated the national standards with Healthy People 2010, which continues to address the health needs of communities with an outcome-driven system.
