Answer to Question 1

C
When performing wound irrigation, prepare the sterile irrigation tray and dressing supplies and pour room-temperature solution into the sterile container. This maintains the sterility of the procedure.

Answer to Question 2

B
Ectopic pregnancy occurs when the fertilized ovum implants outside the uterus. The pregnancy appears normal until 3 to 5 weeks after the first missed period, when the client experiences a dull ache on one side of the abdomen that becomes increasingly severe. When the tube ruptures, the client will have a single excruciating pain in the abdomen. Referred shoulder pain is common. Serial testing of HCG will show a lower-than-normal level. Surgery must be performed to prevent a rupture of the fallopian tube.
