Answer to Question 1

C
When applying a vaginal medication, encourage the client to apply a perineal pad to collect any discharge or drainage to prevent soiling of garments and skin irritation if medication comes in contact with other areas.

Answer to Question 2

D
Hyperemesis gravidarum is excessive vomiting during pregnancy, which leads to dehydration, fluid and electrolyte imbalance, and alkalosis. Low potassium may cause cardiac problems, and the client may experience metabolic changes or even death. Fetal death may also occur. The client is usually hospitalized and given IV therapy and sedatives and is kept NPO until all vomiting has stopped for 48 hours.
