Answer to Question 1

B
Feedback:
Because a tracheostomy tube is below the level of the larynx, clients usually cannot speak. A tracheostomy does not necessarily create a risk for fluid volume deficit. Functional deficits are unlikely to be unilateral and the client's possible decisional conflict is not likely to be a direct consequence of the tracheostomy.

Answer to Question 2

C
Feedback:
The nurse should encourage elderly clients to use relaxation techniques before bedtime such as chair rocking, imagery, meditation, deep breathing, progressive relaxation, soothing music, body or foot massage, reading nonstimulating materials, or watching nonstimulating television. Alcohol should not be used to produce sleep and a crossword puzzle may be too stimulating. The client should dress in warm, not cool, clothes.
