Answer to Question 1

2,3,5
Rationale: The nurse identifies objective data during the evaluation of comfort, but this is not usually a consideration when outcomes are not achieved.

Answer to Question 2

3
Rationale: The best route for measuring temperature would be tympanic. Oral is contraindicated because the client is drinking hot coffee. Axillary would be less accurate than would tympanic. Rectal would be embarrassing for the client, and is not required in order to get an accurate temperature.
