Answer to Question 1

2
Rationale: It is very important to elderly clients to maintain their independence and they should be included in all care planning to help them achieve this goal. Not all elderly clients have difficult financial situations. Preparation for death would not be planned unless there were a reason to anticipate that this is impending. Social opportunities are more important for the adolescent than for the elderly client, and not nearly as important as maintaining independence.

Answer to Question 2

1,2,3,5
Rationale: Urticaria is a localized rash which is generalized over the body
