Answer to Question 1

2
Rationale 1: When using the nursing process as a model for making decisions, this question can be asked during planning for and implementing the nursing diagnosis.

Answer to Question 2

1
Rationale 1: The tick's infecting the client is an example of a vector-borne method of transmission. The connection to the circulatory system allowing the infection to pass into the client is the portal of entry. If the client wasn't feeling well, this would increase the susceptibility of the host. Washing hands would have no impact on reducing the risk of a tick, and would demonstrate the concept of a reservoir of other types of infection.
