Answer to Question 1

4
Rationale: Repeated injections in the same area contribute to tissue ischemia and the formation of pressure ulcers, but are not one of the most important risk factors

Answer to Question 2

1
Rationale: Pregnant women should be taught to take their prenatal vitamins to avoid deficiencies, and to avoid eating foods high in mercury in order to reduce the risk of fetal damage. Someone else should clean the cat's litter box, so the pregnant woman can avoid exposure to toxoplasmosis. Safe levels of alcohol intake have not been established, so the pregnant woman should avoid all alcohol intake, and the seat belt should be placed low on the pelvis to avoid fetal harm.
