Answer to Question 1

C
Process indicators are quality indicators that evaluate the manner in which care is delivered.
Structure indicators evaluate the structure or systems for delivering care; an example is adherence to checking that emergency carts are adequately stocked.
Team indicators do not exist in this context.
Client indicators would actually be outcome indicators. Outcome indicators evaluate the result of care delivered.

Answer to Question 2

B
Avoid crushed medication if liquid is available. Irrigate with 30 mL of water before and after each medication per tube. Dilute crushed medications if not liquid. Read pharmacological information on compatibility of drugs and formula before mixing medications.
