Answer to Question 1

D

Feedback
A Documenting the unsafe practices is not the initial action to undertake.
B Completing a physical assessment is not the initial action to undertake, although it may be the appropriate follow-up to an assessment of the circumstances that contribute to the unsafe practices.
C Deciding what strategies would work best for the client is not the initial action to undertake and does not reflect the collaborative nature of community health nursing.
D The nurse should initially partner with the client to assess the circumstances that contribute to substance use, unsafe sexual practices, or other risk behaviours, and identify strategies to address clients' often multiple and interrelated concerns.

Answer to Question 2

C
No topic that a dying client wishes to discuss should be avoided. The nurse should respond to questions openly and honestly. As client advocate, the nurse should assist the client to obtain the necessary information to make this decision.
The nurse should provide the client with information with which to make such a decision. Alt-hough the nurse may suggest that the client discuss the subject with the family after having ob-tained information, it is up to the client to discuss the subject with his family.
The nurse should respect the client and provide the necessary information for him or her to make a decision, rather than dismissing the client's question.
It is not necessary to contact the physician or the family for consent for organ donation if the client is capable of making this decision.

Although the Roman numeral for the number 4 has always been taught to have been "IV," according to historians, the ancient Romans probably used "IIII" most of the time. This is partially backed up by the fact that early grandfather clocks displayed IIII for the number 4 instead of IV. Early clockmakers apparently thought that the IIII balanced out the VIII (used for the number 8) on the clock face and that it just looked better.
