Answer to Question 1

B
The nurse should instruct the male client that the protocol for testicular self-examination is to use both hands to roll the testicle gently, between index and middle fingers, feeling for lumps, thick-ening, or a change in consistency (hardening).
All men 15 years and older should perform the testicular self-examination.
The examination should be performed after a warm bath or shower when the scrotal sac is re-laxed.
A cordlike structure on the top and back of the testicle is a normal finding; this is the epididymis.

Answer to Question 2

1 in 4 clients is not the documented number of clients.
1 in 50 clients is not the documented number of clients.
