Answer to Question 1

B
Many nurses avoid acknowledging a patient's pain because of their own fear of contributing to addiction. These fears and beliefs lead to mistrust between the nurse and patient, increased pa-tient recovery time, increased complications and mortality, increased psychological problems, and increased cost.

Answer to Question 2

D
The act Hospital-Acquired Conditions and Present on Admission Indicator Reporting identified eight conditions that are preventable through the application of evidence-based guidelines: object left in surgery, air embolism, blood incompatibility, catheter-associated urinary tract infection, pressure ulcers, vascular catheter-associated infection, surgical site infection after coronary artery bypass graft surgery, and falls and trauma. The government will not financially reimburse acute care hospitals if a patient acquires one of these eight conditions during hospitalization.
