The definition of chronic pain involves the length of time that pain is experienced and the pro-gressive nature of the problem causing the pain. Osteoarthritis meets these criteria. A headache caused by fluid loss during a spinal tap is severe but has a duration of only 3 to 5 days. Hip re-placement surgery, although resulting in postoperative pain, is time limited and often relieves chronic hip pain. IV needle insertion is considered painful by some patients, but is short proce-dural pain.

When a calcium channel blocker such as diltiazem is prescribed for an older patient, or a patient with hepatic (liver) or renal (kidney) impairment, lower than normal initial doses are prescribed. The nurse should hold the drug and contact the prescriber about the dosage that has been pre-scribed for this patient.
