Answer to Question 1

D

Answer to Question 2

D
A, B, C, and D. Examination of the lungs and thorax is most effective when the client is undressed to the waist. Begin with the client sitting for assessment of the posterior and lateral chest. The client may sit or lie down for examination of the anterior chest. A female client may keep a gown draped loosely over her chest while the posterior chest is examined. Good lighting is essential. The nurse needs to assess the client's ability to tolerate position changes and level of distress. Often a client confined to bed rest or the client with chest pain has limited lung expansion.
