D. Be sure receptacle for solution is located near table/work surface edge. Sterile kits have cups or plastic molded sections into which fluids can be poured. Prevents reaching over sterile field during pouring of solution.
A. Being sure wrapper does not fall down on sterile field, place item onto field at an angle. Secured wrapper edges prevent flipping wrapper and contaminating contents of sterile field.
B. First open outermost flap away from body, keeping arm outstretched and away from sterile field.
C. Apply cap, mask, protective eyewear, and/or gown as needed (consult agency's policy). Perform hand hygiene thoroughly using an alcohol-based hand rub or an antimicrobial soap and water.

D. Ask client to remain sitting upright for at least 30 minutes after the meal to reduce the risk of gastroesophageal reflux, which can cause aspiration.
A. Client must be sitting upright for the passage of food through the pharynx and esophagus.
B. Observe client's ability to ingest foods of various textures and thicknesses to indicate whether aspiration risk is increased with thin liquids.
C. Observe client with various consistencies of liquids. Difficulty managing certain foods may indicate dysphagia and referral to a dietitian is appropriate if a client has difficulty with a particular consistency.
