Answer to Question 1

The external equalizing line should be connected to the top of the tube.

Answer to Question 2

It is important to understand that the torch is not used to directly melt the solder. To insure that both the base metal and the solder are hot enough to bond, the torch should only be used to heat the metal and the metal should melt the solder.
