Answer to Question 1

A radiation-related incident must be reported if it meets a specific set of requirements. An incident is defined by OSHA as follows:
Exposure of the whole body of any individual to 25 rems or more of radiation; exposure of the skin of the whole body of any individual to 150 rems or more of radiation; or exposure of the feet, ankles, hands, or forearms of any individual to 375 rems or more of radiation. The release of radioactive material in concentrations which, if averaged over a period of 24 hours, would exceed 5,000 times the limit specified. If an incident meeting one of these criteria occurs, the employer must notify the proper authorities immediately.

Answer to Question 2

The concrete is poured (cast) at the job site (in place) into forms that are either constructed or assembled at the site. It involves building or assembling forms at the job site; placing steel reinforcing bars into the forms; pouring concrete into the forms and treating the surface of the concrete in some manner, such as applying broom strokes or using a trowel.
