For a number of reasons; they may have nothing to do with the case, they may be irrelevant, inaccurate, etc.

Answer to Question 2

Because the mentioning of insurance may imply that the defendant will be able to cover any losses by having the insurance company pay for the damages; in a sense it is taking the burden of responsibility off the defendant and placing it upon the insurance company to pay any monetary damages.
