 It contains a description of the referral incident, the youth's past conduct, prior contacts with the police and courts, and family environment and relationships.
 Information on the youth's employment history, school records, interests, and activities is also collected.
 If police officers have information on special problems of the youth or if special needs of the youth are discovered, these are included in the report.
 It draws up a plan for what the youth needs and how he or she can be best helped.
