Exhaustive means that each question includes all possible alternatives as choices. If a question is not exhaustive, respondents may be forced to select an answer that doesn't reflect their true thoughts.
Mutually exclusive means that there are no overlapping categories to answer each question. If a question is not mutually exclusive, it will confuse the respondent and yield unreliable survey results.
