Answer to Question 1

The employment-at-will doctrine became the norm in 19th century American common law.

Answer to Question 2

The most common exception to the employment-at-will rule is the public policy exception. Under this although the employee is employed at-will, termination is illegal if a clear and significant mandate of law (statutory or common) is damaged if the firing is permitted to stand unchallenged.
