Answer to Question 1

ANS: D
Decreased arterial oxygen saturation is characteristic of secondary polycythemia. Arterial oxygen saturation is normal in polycythemia vera. Decreased arterial oxygen saturation leads to hypoxia, resulting in increased production of erythropoietin. This stimulates the bone marrow to increase red cell production (erythroid hyperplasia), also found in polycythemia vera. This results in an increase in red cell mass, again also found in polycythemia vera, as is an increased white count.

Answer to Question 2

ANS: A
Thrombocytosis, or an elevated platelet count, is characteristically present in polycythemia vera. Polycythemia vera has an elevated red cell mass, so anemia is not present. The leukocyte alkaline phosphatase score and the white count are both elevated.

Chronic necrotizing aspergillosis has a slowly progressive process that, unlike invasive aspergillosis, does not spread to other organ systems or the blood vessels. It most often affects middle-aged and elderly individuals, spreading to surrounding tissue in the lungs. The disease often does not respond to conventionally successful treatments, and requires individualized therapies in order to keep it from becoming life-threatening.

Once thought to have neurofibromatosis, Joseph Merrick (also known as "the elephant man") is now, in retrospect, thought by clinical experts to have had Proteus syndrome. This endocrine disease causes continued and abnormal growth of the bones, muscles, skin, and so on and can become completely debilitating with severe deformities occurring anywhere on the body.

Prostaglandins were first isolated from human semen in Sweden in the 1930s. They were so named because the researcher thought that they came from the prostate gland. In fact, prostaglandins exist and are synthesized in almost every cell of the body.
