Any two of the following:
 Incomplete gathering of patient information. This error may happen when the healthcare professional does not know about patient allergies, other medications the patient is taking, previous diagnoses, or lab results.
 Unavailable drug information. This error may happen when the prescribing provider or pharmacist is unaware of up-to-date warnings about medications or contraindications.
 Miscommunication of drug orders. This type of error can occur with poor handwriting in the patient chart or on the prescription, confusion between drugs with similar names, misunderstanding of where zeroes and decimal points lie, confusion of metric with other dosing units, as well as or use of inappropriate abbreviations.
 Lack of appropriate labeling. This error happens most commonly when drugs are prepared or drawn up.
 Environmental factors. Poor lighting, noise, interruptions, or other environmental factors that distract healthcare professionals can lead to this type of error.
