Gross hematuria indicates bleeding from some point in the urinary tract. If the blood is noticed as soon as voiding starts, it is likely that the blood is from somewhere in the urethra. Blood that ap-pears at the end of urination probably comes from near the neck of the bladder. Bleeding throughout voiding indicates that the blood is coming from a site above the neck of the bladder because the blood has been well mixed with the urine in the bladder.

Answer to Question 2

The patient is most likely experiencing a ruptured aneurysm, which is a medical emergency re-quiring surgical repair. The nurse should contact the physician immediately. The vital signs may need to be measured more often than every 15 minutes. Notifying the family is not the priority intervention. Ongoing assessment of pain should continue, but after the physician is notified of the emergent status change.
