Answer to Question 1

1, 2, 4, 5
Advance directives allow a person to make specific decisions about their future health care treatments in advance. Forms of advance directives are the living will, durable power of attorney for health care, and written or verbal advance directive. The incident report is a document that records errors of omission or commission as well as any unusual occurrence. The medication administration record does not address clients' treatment choices.

Answer to Question 2

2
The client from a non-English-speaking culture could have cultural limitations on the prescribed treatment. The best approach would be for the nurse to talk with the client about the treatment to find out why it is not being accepted. Discharging or transferring the client would not meet the client's health care or cultural needs. Asking the physician to prescribe a different treatment also does not meet the client's cultural needs.

Although the Roman numeral for the number 4 has always been taught to have been "IV," according to historians, the ancient Romans probably used "IIII" most of the time. This is partially backed up by the fact that early grandfather clocks displayed IIII for the number 4 instead of IV. Early clockmakers apparently thought that the IIII balanced out the VIII (used for the number 8) on the clock face and that it just looked better.

Drug abusers experience the following scenario: The pleasure given by their drug (or drugs) of choice is so strong that it is difficult to eradicate even after years of staying away from the substances involved. Certain triggers may cause a drug abuser to relapse. Research shows that long-term drug abuse results in significant changes in brain function that persist long after an individual stops using drugs. It is most important to realize that the same is true of not just illegal substances but alcohol and tobacco as well.
