The nurse is caring for an older client who does not exhibit an elevated temperature but is suspected of having a severe infection. The nurse should look for other manifestations of infection, such as:

  2. change in mental function or delirium.
  3. diarrhea.
  4. nausea and vomiting.

2. In the elderly, confusion is a frequent atypical sign of infection, along with restlessness, fatigue, and behavioral changes.
3. Diarrhea indicates a gastrointestinal problem.
4. Nausea and vomiting can have many causes; however, it is not directly linked to an infection in an older client.

1. Peak and trough levels are not drawn on the cephalosporins.
2. The heart rate is not likely to be affected with administration of this drug unless the client is allergic to it.
3. Cephalosporins can cause kidney damage, and the nurse should monitor output and BUN in older clients.
4. Blood pressure is not usually affected by these drugs.
