2. Capillary refill of the nail beds is monitored as an indicator of cardiac output.
3. Paresthesia is numbness around the mouth and in the toes and fingers, and occurs with metabolic alkalosis.
4. Hyperactive reflexes are seen with metabolic alkalosis.

Answer to Question 2

1. One bath a day is sufficient for this client.
2. Normal routine oral care is planned.
3. Ambulation might not be possible until the client's magnesium is higher.
4. This client will experience muscle weakness, dysphagia, and possibly seizures. The nurse should plan interventions to prevent injury in this client.
