Answer to Question 1

Answer: 2

1. Unless the client is experiencing severe memory loss or dementia, it is not necessary to have someone check his medications.
2. Many over-the-counter medications and herbals can potentiate or decrease the effect of prescription medications. The client should be instructed to discuss all medications with the primary care provider.
3. The client should be instructed to notify the physician of side effects of medications taken.
4. Some medications are best taken with food, while others are taken on an empty stomach.

Answer to Question 2

Answer: 3

1. Failing eyesight has nothing to do with the appropriate water temperature for the older client.
2. Hot water does not have an effect on urinary incontinence.
3. The skin of the elderly is fragile and will burn more easily than that of a younger person.
4. Cold water will chill the elderly client but will not cause frostbite.
