1. Due to a reduction of muscle mass and other factors, the elderly client is at risk for injury. The nurse should assist older clients in assessing the home for safety and the reduction of potential injury hazards.
2. Increasing exercise will depend upon each individual's physical status. If not performed appropriately, the older client can experience injury for over-exercise.
3. Urinary incontinence is not one of the physical changes expected in the older adult.
4. The older adult is encouraged to maintain an adequate fluid intake, not a decrease.

1. The nurse would want to help the client to increase exercise during the day to promote better nighttime sleep.
2. Increasing the amount of fluids at night would likely disrupt sleep at night because the client would be using the bathroom more.
3. Pain is one of the main issues that decreases quality sleep and contributes to fatigue.
4. Pain relief would be more helpful than sleep medication, and should be tried first.
