Answer to Question 1

Answer: B

Sensory deficits may increase a client's risk for injury, especially falls and burns. The nurse should determine what assistive devices are needed to promote client safety. Teaching how to adapt to the sensory deficit supports the client's self-concept. Providing meaningful interaction and stimulation supports the client's level of stress and coping. Teaching the need to take antibiotics as prescribed supports the potential risk for infection.

Answer to Question 2

Answer: D

The client is recovering from surgery to repair fractured facial bones. It is likely that the client's sense of smell and taste have been altered by the injuries and surgery. The nurse should explain that the facial injuries are affecting the client's sense of taste and flavor. The nurse should not order something else but explain that the client's sense of taste and flavor is going to be different for many foods until the injuries heal. The response that the meal is the best the cafeteria has to offer is inappropriate. Telling the client not to eat anything the client does not want is also inappropriate.
