Answer to Question 1

Answer: D

Since the client is recovering from an appendectomy, the client will most likely have acute pain. There is no evidence to suggest the client is at risk for impaired perfusion or deficient fluid volume. Because the appendix ruptured before surgery, the client is at risk for infection, specifically peritonitis.

Answer to Question 2

Answer: B

While families may need education about seat belts and sources of support, it is not the optimal time to implement such teaching at this point in the crisis. It is optimal to find out the family's perception of what is going on and what they feel their needs are. The best way to determine this is to encourage them to express their feelings. Timelines for visitation are appropriate but are not the priority.
