Answer to Question 1

Correct Answer: 4

The unlicensed assistive personnel (UAP) can safely measure oxygen saturation or reposition the oxygen mask on the client's face. Only the nurse should determine the need for increased oxygen delivery, initiation of therapy, or changing the delivery method of oxygen.

Answer to Question 2

Oxygen toxicity can develop as early as 12 hours after breathing high concentrations of oxygen, and is indicated by substernal pain, cough, sore throat, dyspnea, and pulmonary edema. Oxygen toxicity should be ruled out before assessing for other causes.
