The Holmes Social Readjustment Scale is used to measure the stressors in a client's life. The Duke Social Support and Stress scale is an instrument to measure family and nonfamily support and stress. The Emotional Readiness Scale is not a true test, and therefore not a valid test to assess psychosocial variables in a client.

Involving other clients in the plan of care may be a violation of the client's privacy and is not appropriate.
